Job Summary

The Nurse Navigator - Oncology at North Mississippi Health Services plays a crucial role in providing patient care management and coordination for the oncology patient population. The Nurse Navigator also analyzes data, identifies care gaps, and makes recommendations to improve services and patient outcomes.
Job Functions
Patient Care Management:
- Acts as clinical liaison for newly proposed cancer patients.
- Develops and manages processes that directly improve care management and care coordination for the oncology patient population.
- Assists patient care team in providing high quality care in a cost-effective manner.
- Evaluates patient progress and intervenes as necessary to ensure optimal patient outcome.
- Assesses patient benefits and coordinates care.
- Provides education and information to patient/caregiver on diagnosis and resources/services available.
- Keeps patient/caregivers involved in care plan.
- Actively engages patients in self-management and education.
- Reporting:
- Gathers and analyzes data and issues recommendations to improve services/care.
- Coordinates/facilitates case presentation at tumor conferences.
- Develops relationship with patients and provides emotional support.
- Pre-Visit Planning. Identify and contact patients with care gaps prior to their scheduled appointments.
- Coordinate care and ensure results are available prior to appointment with physician.
- Post-Visit Planning. Follow -up with patient to ensure individual plan of care is being followed.
- Assesses patient progress toward individual goals and notify physician when issues arise that prevent patient progress towards goals.
